Chapter 18: Vendor and CRO Management
How delegation, CROs, vendors, oversight, interfaces, and governance really work. ## Delegation Is Not Disappearance DIAB-220 had reached the stage where every outsourced partner claimed to be busy. The clinical research organization (CRO) was managing sites and monitoring. The central lab was transferring endpoint data. The electronic clinical outcome assessment (eCOA) vendor was explaining missing diary files. The interactive response technology (IRT) vendor was reconciling exposure and drug-return records. The safety vendor was working with pharmacovigilance (PV) on serious adverse event reconciliation. The programming vendor was preparing tables, listings, and figures. Everyone was working. That did not mean the sponsor had oversight. Priya Raman, Director of CRO and Vendor Management, joined Lauren Brooks with a sentence Lauren wrote down immediately: > Delegation changes who performs the work. It does not make sponsor accountability disappear. A sponsor is the organization responsible for initiating and overseeing the trial. A CRO is a company contracted to perform clinical trial activities for the sponsor. A vendor or service provider is an external partner that delivers a specific service, system, data stream, logistics function, laboratory service, medical writing support, programming support, recruitment support, safety process, or technology tool. Outsourcing means using external partners to perform work. Delegation means assigning duties to another party under defined expectations. Sponsor accountability means the sponsor remains responsible for appropriate oversight, participant protection, data integrity, reliable results, and regulatory credibility even when tasks are delegated. Lauren had once said, "The vendor owns that." Priya corrected the sentence: "The vendor may own the task. The first was passive outsourcing: waiting for a partner to fix a problem because "they own it." The second was micromanagement: trying to do the partner's technical work from the sponsor side. Sponsor oversight sits between those mistakes. ## The Vendor Landscape Not every external partner is a CRO. The PM should know the interface, not every technical detail. ## Choosing Partners Before The Problem Exists Vendor qualification is documented evaluation of whether a partner can perform the required work. Due diligence is the investigation behind that judgment. A request for proposal (RFP) asks potential partners to describe how they would deliver the work. A bid defense is a vendor presentation where the sponsor tests the proposal, team, assumptions, and fit. Lauren reviewed a low-cost eCOA vendor for a future study. The dashboard looked beautiful. The price looked better. Quality noted weak validation documentation, limited global support, and no clear site help-desk model. "A cheap vendor becomes expensive when the endpoint is missing," Priya said. The team used the quality management plan [PMBOK] to turn the details into operating choices the PM could assign, monitor, and escalate: Capability. What to Test was Can the vendor perform the required service at the needed scale? Experience. What to Test was Has the vendor handled similar trials, populations, countries, and endpoints? Systems.
